How Do I.... Register With The Practice?If you are interested in registering as a patient at either Parkfield Medical Centre or Woodcross Health Centre, please apply directly in person to the reception desk at either site. The reception team will ask for your address details. (If you are able to bring your medical card, this will help us to process your application and to confirm if you are residing with an existing patient on the list.) The receptionist will confirm that your address is within our practice's allocated area, as submitted and held by the Primary Care Trust. Please note - this application is to register with 'Parkfield Medical Practice'. You may express a preference for a particular doctor, which will be indicated on your Registration Form. However, if you want to see a specific doctor, it is likely that you will have to wait longer than 48 hours for an appointment. New Patients |
